About USMLE
The USMLE (United States Medical Licensing Examination) is a three-step examination required for medical licensure in the United States. It is jointly conducted by the Federation of State Medical Boards (FSMB) and the National Board of Medical Examiners (NBME).
International medical students and graduates must also meet additional certification requirements through the Educational Commission for Foreign Medical Graduates (ECFMG) to be eligible.
Who Needs USMLE?
- Students/graduates from India or any country outside the US and Canada intending to:
- Apply for residency programs in the US
- Obtain clinical experience in the US
- Acquire licensure to practice medicine independently in the US
Exam Format and Duration

Note: You can take Step 1 and Step 2 CK in any order. Step 3 is permitted only after passing both Step 1 and Step 2 CK.
Eligibility Criteria for Indian and Foreign Students
Step 1 and Step 2 CK
You must be:
- Enrolled in or graduated from a medical school outside the US and Canada that is listed in the World Directory of Medical Schools (WDOMS)
- Your school must meet ECFMG eligibility criteria
Step 3
You must:
- Have passed Step 1 and Step 2 CK
- Be ECFMG certified
- Hold the equivalent of an MD degree from a recognized medical school
ECFMG Certification (Mandatory for All IMGs)
Key Requirements:
- Medical school listed in WDOMS and must meet ECFMG eligibility requirements
- Successful completion of USMLE Step 1 and Step 2 CK
- Primary source verification of medical school transcripts and diploma
- Application via ECFMG’s IWA portal (Interactive Web Applications)
ECFMG Pathway for 2025
- IMGs applying for 2025 US residency must complete all steps required by ECFMG including:
- Pathways application (if applicable)
- OET Medicine for English proficiency
Application Process (For Students Outside the US/Canada)
Step-by-Step Guide:
- Create an account at ECFMG’s website.
- Submit Form 186 (Certification of Identification Form) signed by your medical school.
- Apply for ECFMG Certification.
- Apply for Step 1 and Step 2 CK through ECFMG’s IWA portal.
- Choose a 3-month eligibility period for each Step.
- Once approved, receive your scheduling permit.
- Book your test at Prometric Centers (available in major cities like New Delhi, Hyderabad, Chennai, etc.).
- Prepare using official USMLE practice materials and NBME self-assessments.

Fees are non-refundable and non-transferable. Additional fees may apply for rescheduling or extensions.
Testing and Scheduling
- Prometric Centers host Step 1 and Step 2 CK internationally.
- Step 3 can only be taken in the US or its territories.
- Book early to get preferred test dates.
- Eligibility extension is allowed once for a fee if you can’t test within the chosen window.
Score Reporting
- Scores are usually available within 3-4 weeks.
- You can access your score reports via OASIS or IWA.
- If applying for residency via ERAS, your scores must be released to NRMP via ECFMG.
Retake Policy
- You may take each Step a maximum of 4 times.
- If you fail 3 times in 12 months, the 4th attempt must be after 12 months from the first attempt and at least 6 months from the last.
- If you pass a Step, you generally cannot retake it unless required for licensure reasons.

- Ensure your passport name matches the name in your ECFMG/USMLE registration.
- Begin the process at least 12-18 months before your intended residency start.
- Practice with NBME assessments and UWorld question banks.

- Using an unlisted or disqualified medical school – always check WDOMS
